What is Assam Arogya Nidhi Scheme ,
Assam Arogya Nidhi Scheme Is Launched By The Assam Health & Family Welfare Department Government Of Assam. Under This Scheme, The Government Provides Assistance Of maximum up to Rs. 3 Lakh For Treatment Of Any Member Of The Family Earning Less Than Rs. 5 Lakh Who Has Undergone Surgery/Implant/Injury Due To Life Threatening Diseases, Or Accident.
Who Is Eligible For Assam Arogya Nidhi Scheme .
• All Indian Citizens Who Are Ordinarily Resident Of The State Of Assam Will Be Eligible Under The Scheme.
• The Income Of The Family Must Be Less Than Rs. 5 Lakh.
• There Is No Age Limit, People Of All Ages Will Benefit From This Scheme.
• Government Employees And Pensioners Are Not Eligible For This Scheme.
• Under This Scheme, The Government Will Provide About maximum Up to 3 Lakh For Treatment.
• The Scheme Covers Atal Amrit Card And Pradhan Mantri Jana Arugya Yojona.
• This Scheme Provides Financial Assistance For (I) Life Threatening Diseases, And (Ii) Injuries Caused By Natural And Man-Made Disasters, Such As Industrial/Agricultural/Road/Railway Accidents, Bomb Blasts Etc. As Well As General And Special Treatment. Cardiology And Cardiac Surgery, Cancer, Kidney And Urinary Diseases, Orthopaedics, Thalassemia, Bone Marrow Transplantation, Aids, And Chronic Mental Illness With Surgery. Government Provides Financial Assistance In Such Matters.
• All Financial Assistance Will Be Admissible For Treatment In All Government Hospitals Including Medical Colleges/ Tertiary Health Care Facilities, All Reputed Private Hospitals Registered Under The Clinical Establishment Act And Empanelled Hospitals Of The Government Of Assam.
Required Documents.
1. Photo Of The Patient Attested By The Doctor.
2. Attested Photocopy Income Certificate Certified By Revenue Circle Officer.
3. Attested Photocopy Of (Adhar Card/Voter Card/ Driving Licence )Identity Proof And Address Proof Of The Patient’s .
4. Discharge Certificate Issued By The Hospital.
5. Attested Photocopy Of Front Page Of Bank Pass Book
6. Attested Photocopy Of Pradhanmantri Jana Arugya Yojona /Atal Amrit Abhijan Card.
7. Original Receipts For Treatment Of The Patient Such As Medicines,Medical Tests, Must Be Submitted.
8. Copy Of The Medical Documents Of Treatment To Be Certified By Treating Doctor/Hospital.
Procedure.
Applicants Should Submit The Scheme Application Form Along With Documents To Assam Mission Director Office. National Health Mission, Documents Can Be Sent By Post. And also You can apply with the help of the Arogya Nidhi Scheme support departments in medical colleges.
After The Committee , doctor, medical College Checks and examination The Documents Of The Patient And If The Patient Is Eligible Financial Assistance Will Be Disbursed To The Beneficiary Concerned Or Reimbursed To The Hospital Concerned Through Bank Account Transfer Only. The Advance Payment Shall Only Be To The Hospital Subject To A Maximum Of ₹3.00 Lakh.
